Please fill out the form below to begin the return process. Upon receipt of the completed form, you will be issued a Return to Stock Number (Return to Stock items are subject to a 30% restocking charge after the 90 day trial period).
You will be refunded for the merchandise in total (shipping and handling charges are not refundable unless product arrives damaged or the incorrect product was shipped). Please allow 14 business days for us to process your return. In addition, allow 1-2 billing cycles for the credit to show up on your credit card statement.
